I will show how to assess and manage PTBS in Psychotherapy Sessions using IS-TDP

Posttraumatic stress disorder (PTSD) as an incapacitating condition the patient experiences after life threatening trauma. Often childhood adverse experiences and attachment trauma underpin and color the picture of PTSD. Intensive short-term dynamic psychotherapy (ISTDP) is a broadly applicable relatively brief treatment method for the full spectrum of patients with posttraumatic stress disorder.

• In this 2 day video-based immersion course we will review the metapsychology of this method, then will detail treatment of two cases with post traumatic stress disorder. One of these cases will have mainly muscle tension and anxiety without prominent dissociation and the other will have dissociative phenomena as a component of fragile character structure.
• These two groups of patients are treated quite differently with the first group being able to directly access the emotions related to their traumatic experiences including their childhood attachment trauma. The second group requires various formats of capacity building so that the anxiety is well-tolerated and the emotions can be experienced in a way that is more beneficial and less distressing to the patient.
• Both of these ISTDP methods can be used to augment traditional cognitive behavioral methods, enabling working with resistant patients and also patients who are experiencing such low anxiety tolerance that they cannot easily benefit from exposure-based methods.

In diesem dreitägigen ISTDP-Vertiefungskurs werden wir eine Reihe von Patienten untersuchen, die einer wirksamen Behandlung von Geist und Körper bedürfen. In diesem Behandlungsformat evaluieren wir die Angstentladungswege, regulieren bei Bedarf die Angst und helfen dem Patienten, wenn möglich einen Durchbruch zu den zugrunde liegenden komplexen Gefühlen zu erzielen, welche die Krise verursachen.

In this treatment format we evaluate the anxiety discharge pathways, regulate anxiety as needed and assist the patient to achieve a breakthrough where possible to underlying complex feelings that are creating the crisis. In this three day course we will review a series of patients who presented in a crisis session to review the assessment and treatment procedures of this method and its utility with diverse populations of patients.

• a moderately highly resistant patient presents with depersonalization and derealization with suicidal thoughts ahead of a major unlocking. This case was treated many years ago and will allow discussion of learning processes over time.
   
• a man with low anxiety tolerance and chronic somatic symptoms, presents with depletion and hopelessness after making some gains ahead of a significant breakthrough.
   
• a patient with chronic major depression regressive defenses and substance dependence, presents with a major upsurge in symptoms after stopping substance abuse.
   
• a patient with borderline personality presents with a dissociative episode with some acting out after having a week of positive regard for herself.  All these cases will have video with English subtitles.

In diesem Behandlungsformat evaluieren wir die Angstentladungswege, regulieren bei Bedarf die Angst und helfen dem Patienten, wenn möglich einen Durchbruch zu den zugrunde liegenden komplexen Gefühlen zu erzielen, welche die Krise verursachen.

• Ein mäßig hochgradig resistenter Patient stellt sich mit Depersonalisation und Derealisation sowie mit Suizidgedanken vor, die vor einem großen Durchbruch. Dieser Fall wurde vor vielen Jahren behandelt und ermöglicht die Diskussion von Lernprozessen im Laufe der Zeit.
   
• Ein Mann mit geringer Angsttoleranz und chronischen somatischen Symptomen stellt sich mit Erschöpfung und Hoffnungslosigkeit vor, nachdem er einige Fortschritte gemacht hat, im Vorfeld eines bedeutenden Durchbruchs.
   
• Ein Patient mit chronischer Major Depression, regressiven Abwehrmechanismen und Substanzabhängigkeit stellt sich mit einem starken Anstieg der Symptome vor, nachdem er den Substanzmissbrauch eingestellt hat.
   
• eine Patientin mit Borderline-Persönlichkeit stellt sich mit einer dissoziativen Episode vor, einiges ausagierend, nach einer Woche positiver Wertschätzung für sich selbst.

Alle diese Fälle werden auf Video mit englischen Untertiteln gezeigt. Es wird auch drei Fälle geben, die während dieses dreitägigen Kurses supervidiert werden.
